This is an official enrollment packet. The information contained in this online enrollment packet should be considered private. 

Our office staff is pleased to serve parents wishing to enroll their child in Longleaf School of the Arts Charter. To enroll a student, you must be the biological parent or court-ordered guardian.

We are now completing enrollment documents online. This process will take approximately 20-25 minutes. To complete the online enrollment packet, we will ask you to upload the required documentation. Please gather all required documentation before starting the packet completion process. You will not be able to complete your enrollment packet without first uploading required documentation. You will need the following for EACH child you wish to enroll:

REQUIRED DOCUMENTS FOR REGISTRATION (must be uploaded to your Family Dashboard)

Each family MUST provide the following documents:

• parent/legal guardian valid photo ID
• current proof of residence - gas/water/electric bill or mortgage/lease statement issued within the last 60 days (if not in parent/legal guardian name a notarized Affidavit of Residence form* may need to be completed)
• certified copy of student's birth certificate
• official copy of student’s immunization records for school nurse to review
• most recent report card or transcript 

• Other documents that may be applicable to your student:
• custody documents and/or marriage license for step-parent
• student’s current IEP or 504 Plan*
• Health Plan/Chronic Health Condition Plan*

* please email Dr. Mazaliyah Morris, Director of Exceptional Children, at morris@longleafschool.com to discuss your student's IEP/504 needs and transition to LSA

• Additionally, students entering LSA from a homeschool may need the following documents:
• NC Health Assessment Transmittal form (only required for students that are entering a North Carolina public school for the first time)
• High School Credit Request form (if requesting high school credit for a course already completed)

REGISTRATION FORMS

• High School Course Credit Request Form (only needed for homeschool students requesting high school credit)
• NC Health Assessment Transmittal Form (only required for students entering a NC Public School for the first time)
• Student Preferred Name Form (only required for students that want to use a preferred name instead of their legal name)
